Home health agency initiatives
Acumentra Health worked with Oregon home health agencies from 2005 through 2008 on Medicare-funded projects to improve management of oral medications and reduce avoidable acute care hospitalizations among patients receiving home care services. Oregon has one of the lowest acute care hospitalization rates in the nation.
Home Health Quality Improvement National Campaign
The Home Health Quality Improvement National Campaign is focused on reducing avoidable hospitalizations and improving medication management. Campaign support includes a website with intervention packages on topics that impact the hospitalization rate, as well as trending data provided to individual agencies that registered to participate.
