Want to help your patients, improve care coordination, and meet a Meaningful Use objective?
Join the IIS Innovation Project!

Acumentra Health, the Medicare Quality Improvement Organization for Oregon, is seeking Oregon medical practices to receive our assistance in the IIS Innovation Project. Oregon is one of two states in this national demonstration, which aims to increase electronic reporting to state immunization registries and to increase adult immunization rates.

Best practices and lessons learned in the Oregon and Wisconsin projects will inform future nationwide programs to assist providers with EHR and IIS interoperability. CMS funding permits us to provide assistance to practices at no fee.

Support for coordinated care initiatives

As use of EHRs becomes the norm for practices and healthcare facilities, the EHR’s ability to exchange data with the IIS will be a critical element of coordinated patient care. This project supports many other efforts, both locally and nationally, with a focus on health information technology, preventive medicine, and coordinated care, such as the CMS EHR Incentive ProgramsOregon’s health information exchange efforts, and the Patient-Centered Primary Care Home Program.
