Oregon Learning and Action Networks (LANs)

Aim: Improve Health for Populations and Communities

Do you want no-charge assistance to help your practice

  • qualify for Meaningful Use, Patient-Centered Primary Care Home, and other payment incentives?
  • increase revenue through efficiency?
  • improve patient care and outcomes, using Million Hearts and preventive care measures?

The Oregon LANS can support you in reaching your goals! Learning and action networks (LANs) are communities of practice that use an “everyone teaches, everyone learns” approach to maximize the spread of improvement methods and successful interventions.

Cardiac Health Network: Focuses on improvement and EHR reporting of the national Million Hearts ABCS measures to improve the health of cardiac patients)

Preventive Care & HIT Network: Assists REC-enrolled practices that have achieved Meaningful Use Stage 1. Acumentra Health also supports activities in Oregon that integrate HIT to achieve meaningful use of EHRs and improve care coordination and preventive care delivery.
