Integrating Care for Populations and Communities

As Medicare’s Quality Improvement Organization for Oregon, Acumentra Health leads community-based efforts to integrate care Medicare beneficiaries receive as they transition between settings of care. One facet is support for Community-Based Organizations (CBOs) that are applying for funding under the federal Community-based Care Transitions Program (CCTP), a demonstration project that aims to reduce hospital readmissions, test sustainable funding streams for care transition services, maintain or improve quality of care, and document measureable savings to the Medicare program.

Acumentra Health hosted a webinar on October 27, 2011 to provide information on the CCTP application process and tools to help potential CBOs gauge their preparedness and complete an application.

Oregon communities developing transitional care programs can find comprehensive support online in a toolkit of analysis and intervention materials, recorded webinars, and model documents created as part of the 14-state pilot program. The website is hosted by the Colorado QIO (CFMC), which led the pilot program and is coordinating the current national project.
