Engaging forward-thinking Oregon nursing homes in a unique participatory learning community
Goal
Participating nursing homes will work toward establishing a sustainable culture of person-centered care that ensures the physical and emotional well-being of residents.
Areas of focus
Nursing homes will focus on two major areas:
- Recruiting and retaining qualified frontline staff through person-centered care practices
- Improving systems of care that minimize or eliminate the need for physical restraints, prevent and treat pressure ulcers, improve management of chronic pain, and identify and treat depression
Intervention strategy
Nursing homes participating in the Network will apply a strategic approach:
- Set annual target performance rate on the STAR website for the four publicly reported measures
- Access and apply best practices and tools to improve performance on the publicly reported measures
- Conduct annual resident and staff satisfaction surveys
- Collect and monitor certified nursing assistant turnover rates
- Attend Network educational events
- Share successful approaches with others
