National initiatives

2007 Home Health Quality Improvement National Campaign

Statewide initiatives

Statewide, home health agencies are working to improve

• acute care hospitalizations
• management of oral medications
• screening and follow-up for flu and pneumococcal immunizations

Fast Track initiatives

In addition, two Fast Track groups are accelerating improvement in reported measures.

Focus of Home Health Fast Track #1

• acute care hospitalizations
• management of oral medications (or an alternative publicly reported measure)

Focus of Home Health Fast Track #2

• reducing acute care hospitalization through
  telehealth technologies and processes
• systems improvement and organizational culture change

Medicare information

• Publicly reported home health outcome measures (revised fall 2005)
• Medicare’s Home Health Compare website
