- acute care hospitalizations
- management of oral medications
- screening and follow-up for flu and pneumococcal immunizations
Fast Track initiatives
In addition, two Fast Track groups are accelerating improvement in reported measures.

- acute care hospitalizations
- management of oral medications (or an alternative publicly reported measure)
Focus of Home Health Fast Track #2
- reducing acute care hospitalization through
telehealth technologies and processes - systems improvement and organizational culture change
